All of us may face a variety of challenges as we grow older, especially when we experience life or health transitions and corresponding changes in our mental health, substance use, and cognition. These challenges are often exacerbated by isolation from support and resources, leading to potential crisis and increased use of emergency services.  

The Geriatric Regional Assessment Team (GRAT), a program of Sound Generations, employs a team of behavioral health professionals who interface with adults aged 55+ living in King County and experiencing one or more behavioral health issues. We reach out to referred older adults at their homes and strive to complete a comprehensive biopsychosocial needs assessment that can lead to tailored referrals and connection to support services and resources. By engaging older adults when they show signs of being at risk, our team acts to provide early intervention within a short timeframe that can reduce the likelihood of acute behavioral health crisis and the need for emergency services. 

GRAT does NOT provide emergency or crisis services.  
GRAT does NOT provide case management services.  

We honor the autonomy and dignity of all adults. While we always strive to create rapport and engage a referred person, if they clearly refuse our assistance, we will not force participation. At best, we can attempt to offer alternative options and/or guidance to any existing sources of support.  

For additional information about basic program eligibility requirements, please read through our FAQ section below before beginning a referral for the first time. While we want to help as many people as possible, we unfortunately do have some restrictions on eligibility due to our funding contract with King County.

To make a referral, please complete the online referral form. This form is intended for professional use only. If you are a concerned community member, family member, friend, or neighbor, please call the GRAT phone line to make a referral with our Intake Coordinator at 206-448-5730.
